Job Description Job Description A Registered Home Health Nurse
(RN) provides skilled nursing care to patients in their homes,
promoting health, managing chronic conditions, and supporting
recovery from illness or surgery. This role emphasizes
patient-centered care, independence, and education while
coordinating closely with physicians, caregivers, and
interdisciplinary healthcare teams. Key Responsibilities: Conduct
comprehensive patient assessments and develop individualized care
plans Administer medications, injections, IV therapy, and wound
care as prescribed Monitor patient progress and document clinical
findings accurately Educate patients and families on disease
management, medications, and safety Coordinate care with
physicians, therapists, and other healthcare professionals Identify
changes in patient condition and respond appropriately Ensure
compliance with home health regulations, policies, and standards of
care Provide compassionate support while promoting patient
independence and quality of life Complete start of care visits,
recertification visits, and discharge visits. Qualifications:
Active Registered Nurse (RN) license in the state of practice
Associate or Bachelor’s degree in Nursing (BSN preferred) Strong
assessment, critical thinking, and communication skills Ability to
work independently and manage time effectively Valid driver’s
license and reliable transportation Current CPR/BLS certification
Work Environment: Care provided in patients’ homes and community
settings Requires travel between patient locations Flexible
