Pathway Family Services is an affiliate in Kansas offering
additional services to youth and families. Mainstream Nonprofit
Solutions is an affiliate supporting the TFI Family of Companies
and other Nonprofits. As an Aftercare Specialist, you will promote
the well-being of children in the Permanency Services Program,
participates in staff meetings and trainings and coordinates with
staff from other agencies involved in providing services. Utilizing
best practice standards, supports the Case Management team in
managing a caseload of post-permanency placements, with focus on
cases at risk of disruption.
Coordinates all permanence including reintegration, independent
living, adoption and custodianship, as defined by grant during the

agencies involved in providing services/support.
Maintains the accuracy, content and completeness of child/family
Completes documentation and forwards reports containing
descriptive, analytical and evaluative content. Includes a
child/family specific monthly report to DCF.
Coordinates with interdepartmental staff, and staff from external
public and private agencies on the delivery of services to children
The employee is required to be able to safely operate a motor
vehicle and possess a valid license therefore.
Will be required to drive personal or agency vehicle while
transporting children or conducting agency business.
Refrains from assuming any duty that is unrelated to and/or
interferes with the responsibilities of the position.
